Job Summary
The Care Manager Assistant supports the coordination of discharge planning and care transitions under the supervision of a Case Manager. This role serves as an interdisciplinary team member and contributes to efficient care delivery by coordinating post-discharge services, managing documentation, and facilitating communication between patients, families, and healthcare providers. The Care Manager Assistant performs responsibilities in a manner that reflects sensitivity to patient needs and supports quality outcomes aligned with hospital goals.

The Care Manager Assistant supports the coordination of discharge planning and care transitions under the supervision of a Case Manager. This role serves as an interdisciplinary team member and contributes to efficient care delivery by coordinating post-discharge services, managing documentation, and facilitating communication between patients, families, and healthcare providers. The Care Manager Assistant performs responsibilities in a manner that reflects sensitivity to patient needs and supports quality outcomes aligned with hospital goals.
Essential Functions
- Assists with discharge planning by coordinating post-discharge services, such as home health, transportation, or durable medical equipment.
- Communicates with patients, families, providers, and external organizations to ensure timely and appropriate care transitions.
- Maintains accurate and organized records, performs data entry, and processes necessary paperwork related to patient care coordination.
- Schedules follow-up appointments, transportation, or other services as directed by the Case Manager.
- Facilitates and tracks clinical reviews and authorization requests with payors by ensuring accurate and timely delivery of clinical documentation.
- Supports quality improvement initiatives aimed at enhancing care coordination and patient satisfaction.
- Performs other duties as assigned.
- Complies with all policies and standards.
- 1-3 years of experience in a healthcare or patient services role required
- Demonstrated knowledge of medical terminology and basic understanding of ICD-10 and CPT coding.
- Proficient in electronic medical record (EMR) systems and basic computer applications.
- Strong communication skills, both oral and written, with the ability to interact professionally with diverse stakeholders.
- Effective organizational and time management skills; ability to manage multiple tasks simultaneously.
- Demonstrates empathy and support when working with patients and families.
- Strong problem-solving skills and ability to work in a fast-paced environment.
