Job Summary

The Assessment Referral Coordinator supports patient care by coordinating referrals, authorizations, and follow-up appointments in collaboration with physicians and other care team members. This role ensures timely scheduling, accurate documentation, and clear communication with patients to facilitate continued access to services and high-quality care.

Essential Functions
  • Coordinates patient referrals, authorizations, and appointments for specialty care, diagnostics, and follow-up services in accordance with provider orders.
  • Communicates with patients and families to explain next steps, provide instructions, and ensure understanding of referral and appointment requirements.
  • Obtains prior authorizations and referral approvals as needed, and accurately documents information in the electronic health record.
  • Schedules appointments and follows up with external providers to confirm referral completion and collect consult notes.
  • Responds to incoming calls and referral requests, triaging appropriately and ensuring timely processing.
  • Maintains accurate and organized patient records, ensuring documentation is complete and submitted according to policy.
  • Performs other duties as assigned.
  • Maintains regular and reliable attendance.
  • Complies with all policies and standards.
Qualifications
  • Associate Degree in Healthcare Administration or related field preferred
  • 0-2 years of experience in a healthcare administrative or referral coordination role required
Knowledge, Skills and Abilities
  • Knowledge of referral processes, insurance requirements, and prior authorization procedures.
  • Proficiency with electronic health record systems and scheduling software.
  • Excellent verbal and written communication skills.
  • Strong attention to detail and organizational skills.
  • Ability to manage multiple tasks in a fast-paced environment.
